Job Summary

A company is looking for a Care Coordinator I (Clinical Coordination).

Key Responsibilities

• Facilitate execution of clinician-developed care plans and support clinicians with administrative needs
• Act as the first point of contact for clinicians and manage client interactions, including medication authorizations and pharmacy correspondence
• Collaborate with various teams to ensure cohesive client care and assist clients in navigating the healthcare system

Required Qualifications

• Previous experience in a medical or mental health office
• Working knowledge of Electronic Medical Records systems
• 2+ years of hands-on experience supporting clinicians
• Understanding of CPT/ICD 10 coding
• Ability to be flexible in a fast-paced, evolving work environment
