Job Summary

A company is looking for a Per Diem Health Coach.

Key Responsibilities

• Provide comprehensive coaching and support for patients managing diabetes, hypertension, hyperlipidemia, and weight loss through various communication methods
• Create consults aligned with internal protocols and collaborate with healthcare professionals to ensure patient success
• Track patient progress, assess intervention effectiveness, and maintain accurate documentation of care actions and recommendations

Required Qualifications

• Active health coaching or related certification (e.g., NBHWC, Wellcoaches, CDCES)
• Deep understanding of cardiometabolic conditions, particularly diabetes and obesity
• At least 5 years of experience in direct patient care, ideally in clinical or virtual settings
• Familiarity with EMRs and virtual health platforms
• Comfort delivering care virtually using digital tools and maintaining a secure home workspace
