Job Summary

A company is looking for a Care Support Partner to serve as the primary relationship holder within their Care Delivery Service Model.

Key Responsibilities

• Serve as the primary relationship holder and trusted point of contact for clients, families, and caregivers
• Conduct thorough intake assessments and develop individualized service plans
• Coordinate the delivery of services and maintain accurate documentation in the client portal

Required Qualifications

• Bachelor's degree in Social Work, Human Services, Healthcare Administration, or related field
• 3-5 years of direct service experience in a person-centric environment
• Experience in remote care coordination, digital navigation, and case management
• Commitment to serving diverse populations with empathy and respect for cultural differences
• Understanding of health support systems and trauma-informed practices
