Job Summary

A company is looking for a Care Management Associate.

Key Responsibilities

• Place outbound calls to members with acute health conditions
• Educate members on their Care Management program and connect them with a nurse/care manager
• Provide a positive member experience and answer inbound calls from members

Required Qualifications

• 2 years of experience in a customer service call center environment with outbound calling
• High Speed Broadband internet connection (100/25 mbps) required
• Dedicated workspace for remote work during scheduled hours
• High School Diploma or GED
