A company is looking for a Care Coordinator.

Key Responsibilities

• Provide care navigation support for patients, connecting them with providers and social services
• Monitor the member support phone line and address patient inquiries
• Manage patient scheduling, eligibility checks, and referrals to the Case Management team

Required Qualifications

• 4+ years of healthcare administration experience, preferably in virtual care
• Familiarity with social determinants of health (SDOH) platforms and referrals
• Results-oriented with a fast-paced operational approach
• Ability to work cross-functionally with clinical teams
• Passionate about maternal and child health
