Job Summary

A company is looking for a Health Advocate - Part Time.

Key Responsibilities

• Conduct biweekly outreach calls to patients and provide empathetic support
• Coach patients on cardiac care, treatment plans, and lifestyle modifications
• Monitor treatment plan adherence and escalate clinical concerns to the team nurse

Required Qualifications

• Minimum of 2 years of experience as a medical assistant, preferably in cardiac care
• Medical Assistant certification is preferred
• Knowledge of cardiac conditions, treatments, and medications
• Comfortable with virtual telehealth platforms and basic computer skills
• Ability to work collaboratively with a multidisciplinary healthcare team
