Job Summary

A company is looking for a RN, Care Management.

Key Responsibilities

• Perform initial and annual telephonic assessments for Medi-Cal/Medicare dually eligible members
• Ensure clinical appropriateness of member-centered care plans and participate in interdisciplinary planning meetings
• Provide education and coaching for chronic conditions to promote member engagement and self-management

Required Qualifications

• Bachelor's Degree in Nursing required
• Current and active Registered Nurse (RN) License required
• 3+ years of experience in managed care or case management
• 2+ years of experience working with seniors and remotely preferred
• Bilingual Spanish strongly preferred
