Bilingual RN Care Coach

Job Summary

A company is looking for a Bilingual Registered Nurse Care Coach to work remotely and support patients in Medicare's Chronic Care Management Program.

Key Responsibilities:

• Utilize specialized care management software to conduct monthly calls with Medicare patients with chronic conditions
• Build rapport and coach patients on self-management strategies and health improvement goals
• Implement and improve the Plan of Care while connecting patients with necessary community resources

Required Qualifications, Training, and Education:

• Fluent in English and Spanish
• Current, unrestricted RN Compact License or New Mexico RN license
• 3+ years of experience as a Registered Nurse
• Proficiency with EPIC electronic health record and web-based applications
• Self-directed with strong organizational and time management skills
