Job Summary

A company is looking for a Care Manager RN - Population Health.

Key Responsibilities:

• Coordinates and monitors health needs of patients under value-based care risk arrangements
• Develops and implements individualized care plans to optimize health outcomes
• Collaborates with healthcare providers to ensure patient-centered care and appropriate follow-up

Required Qualifications:

• Associates Degree required; Bachelor's Degree preferred
• 3 years of nursing experience required; experience in care management preferred
• Licensed Registered Nurse or Multi-State License required upon hire
• Basic Life Support certification required upon hire
• Residency in service area preferred upon hire
