Registered Nurse Care Management Manager

Job Summary

A company is looking for a Manager of Care Management.

Key Responsibilities:

• Manage daily operations of Care Management teams, including staff hiring, scheduling, and performance evaluation
• Develop and implement workflows and metrics to monitor productivity and ensure efficient team operations
• Oversee staff training and collaborate on the development of educational programs while ensuring compliance with regulations

Required Qualifications:

• Bachelor's degree in nursing or an Associate's degree in Nursing with relevant work experience, or a Master's Degree in Social Work with equivalent experience
• 3 years of prior management experience
• 10 years of clinical experience in acute care, care management, or discharge planning
• Current state licensure as a Registered Nurse
• CCM certification preferred
