Key Responsibilities

• Provides support for clinical outcome management and healthy lifestyle information to patients
• Completes transitions of care calls and arranges follow-up services for moderate risk patients
• Utilizes Motivational Interviewing techniques to engage patients in self-management and monitors care plan outcomes

Required Qualifications

• Certificate Program Post High School Vocational/Specialized Training Required
• 1 year of healthcare experience required
• Licensed Practical Nurse or Multi State License Upon Hire Required
• Basic Life Support certification Upon Hire Required
• Completion of required education programs and competency testing Upon Hire Required
