PA Licensed Behavioral Health Case Manager

Job Summary

A company is looking for a Case Manager for Behavioral Health, specializing in ABA services, to work remotely in the Tri-State Area.

Key Responsibilities:

• Engages members with Autism Spectrum Disorder to address challenges and identify barriers to care
• Collaborates with members and their families to develop personalized care plans and provides follow-up support
• Acts as a liaison within the member's healthcare team, especially in complex behavioral health situations

Qualifications:

• Active PA Registered Nurse (B.S.N. preferred), LCSW, LSW, LMFT, LPC, or BCBA required
• 3-5 years of post-licensure experience in an integrated care setting
• 3 years of experience working with individuals with autism spectrum disorder
• Knowledge of ASD, DSM-5, and ASAM
• Strong understanding of Social Determinants of Health and Motivational Interviewing techniques
