North Carolina Transition Care Coordinator

Job Summary

A company is looking for a Transition Care Coordinator to provide care coordination for individuals with complex needs in the Transitions to Community Living Initiative.

Key Responsibilities

• Collaborate with licensed Clinicians, Registered Nurses, and Peer Support Specialists for discharge and transition planning
• Provide care management to individuals with identified behavioral and physical health needs
• Complete assessments and ensure accurate documentation of transition planning efforts

Required Qualifications

• Master's degree in a human services field or RN licensure with 1 year of relevant experience; or Bachelor's degree with 3 years of relevant experience
• Experience working with individuals with serious emotional disturbances (SED) or serious mental illness (SMI)
• Valid driver's license required
• Must reside in North Carolina and be able to travel within the catchment area
