Job Summary

A company is looking for a Social Worker II - Behavioral Health - Remote Contract.

Key Responsibilities

• Provides culturally competent direct service to patients and families using various social work modalities
• Conducts assessments to identify patients in need of social work intervention and advocates for their access to healthcare
• Formulates and implements treatment and discharge plans while maintaining professional patient care documentation

Required Qualifications

• Master of Social Work required
• Current License to practice as a Clinical Social Worker in New York State required
• 1-3 years of relevant experience required
• Specialized certifications as needed
