New York Licensed Clinical Care Coordinator

Job Summary

A company is looking for a Clinical Care Coordinator (RN) - Remote.

Key Responsibilities

• Provide care coordination for assigned patients and comply with related policies and procedures
• Assist with the development and implementation of care coordination process standards
• Engage patients in managing their health and coordinate follow-up care and referrals

Required Qualifications

• Licensure as a Professional Registered Nurse in New York State with current registration
• BSN or graduate of an accredited RN program with a Bachelor's degree in a related health care field
• Three to five years of clinical experience in ambulatory care, managed care, or acute medical-surgical settings
• Proficiency in managing electronic medical records, such as Epic
• Demonstrated strengths in clinical leadership and collaboration with healthcare teams
