Job Summary

A company is looking for a Nurse Navigator - Physician Access Line.

Key Responsibilities

• Work with the Medical Director and other physicians to identify and carry out treatment plans
• Act as a Patient Advocate to help navigate patients through the diagnosis, treatment, and rehabilitation process
• Provide education for the community and referring physicians

Required Qualifications

• Bachelor's Degree in Nursing
• Registered Nurse (RN) license
• At least 5 years of pediatric RN experience, with 2 years in PICU, NICU, or pediatric ED
• Advanced knowledge in EPIC preferred
• OCN or ONC certification preferred
