Job Summary

A company is looking for a Chronic Kidney Disease - Clinical Nurse, Case and Disease Management.

Key Responsibilities

• Conducts member assessments and develops individualized care plans
• Acts as a liaison among members, employers, and healthcare providers to ensure coordinated care
• Manages transplant cases, including prior authorization and education on transplant benefits

Required Qualifications

• Associate degree in nursing
• 3 years of full-time direct patient care clinical experience in transplant, oncology, or medical/surgical
• Valid and active Registered Nurse (RN) license in Missouri and Kansas
• Ability to understand URAC and NCQA guidelines and state and federal regulations
• Case Management Certification within 3 years of hire date or transfer to Case Management role
