Michigan Licensed Case Manager RN

Job Summary

A company is looking for a Case Manager, Registered Nurse (Remote - Michigan).

Key Responsibilities

• Engage telephonically with members to coordinate their care and develop individualized care plans
• Provide disease management education and connect members with appropriate resources and support services
• Document care management activities accurately and collaborate with the interdisciplinary care team to address barriers to care

Required Qualifications

• Active and unrestricted Registered Nurse (RN) licensure in the state of Michigan
• Proficient in Microsoft Office Suite and healthcare technology tools
• Minimum 3+ years of nursing experience
• Minimum 2+ years of case management, discharge planning, and/or home healthcare coordination experience
• Associate's of Science in Nursing (ASN) degree
