Clinical Navigation, Family Nurse Practitioner (Per Diem)

Included Health (formerly Doctor on Demand + Grand Rounds Health) is seeking a 100% Remote Family Nurse Practitioner in PST or MST to join our team of dedicated mental health clinicians, under the guidance of a Clinical Navigation Manager. This per diem role offers the opportunity to help redefine healthcare by providing compassionate, high-quality virtual care to patients across their healthcare journey. As a Nurse Practitioner, you will connect with members using various modalities to assess their needs and goals, provide clinical guidance, and navigate members to resources to improve their health outcomes. This clinician will work with a multidisciplinary team to help optimize patients' health outcomes.

Easy to use EMR with integrated video streaming capabilities, documentation templates, and patient management systems. 

• Talk with patients, assess their needs, gather important clinical history, provide appropriate clinical guidance, and navigate them to Included Health services, external benefits, and local resources as appropriate.
• Demonstrate strong clinical acumen across a variety of Included Health clinical services, including Expert Medical Opinion, Treatment Decision Support, Healthy Days, and other service lines based on business needs. 
• Maintain Quality of Services: Deliver high-quality clinical services that follow established standards and protocols. Regularly monitor and assess the quality of care provided to ensure it meets or exceeds patient satisfaction and clinical effectiveness benchmarks.
• Evaluate, triage and manage patients across patients for both acute, chronic and mental health conditions.
• Collaborate on pilots, projects, and workflow changes to increase efficiency and improve member experience.

• Be a licensed NP/PA provider in good standing with your state board.
• Maintain board certification
• At least 3 years of NP/PA experience in a clinical setting is required
• Comfortable providing care to all age groups, from newborns to adults.
• Ability to thrive in a dynamic work environment while working independently.
• Must be in PST or MST time zone

• 5+ years of NP/PA experience in a clinical setting is preferred
• Virtual care/telemedicine experience is preferred
• Multi state licensed preferred
• Bilingual is preferred

• Per Diem
• Availability M-F 8am-6pm PST, (12-6pm preferred)

The United States compensation for this part time, per diem position is $63 per hour. Your recruiter will share more about the compensation for this role during the hiring process.
