Job Summary

A company is looking for a Care Management Support Coordinator.

Key Responsibilities

• Perform outreach to members and provide support for care plan next steps and resource connections
• Serve as the front-line support for member and provider inquiries, explaining care plan procedures and protocols
• Document and maintain non-clinical member records in compliance with state and regulatory requirements

Required Qualifications

• High School diploma or GED required
• 1 - 2 years of related experience preferred
• Knowledge of health plan activities and community resources
• Experience with Social Determinants of Health (SDOH) needs
• For specific plans, telephonic interaction with members is required
