Case Manager
As our Case Manager, you’ll work closely with clients to create and implement personalized service plans tailored to their goals, strengths, and needs. Together, you’ll identify self-driven objectives to overcome barriers and build essential living skills, such as grocery shopping, budgeting, meal planning, home upkeep, social skills, coping strategies, and self-monitoring their psychiatric symptoms. Through regular home visits, you’ll guide clients toward achieving their goals while monitoring progress and providing encouragement. You’ll also help connect clients with vital community services and offer transportation as needed to ensure they can access the resources they deserve.

- $17.34-$18.73/hour
- Weekday community-based role- 8:30am – 5:00pm

What we are looking for:
- Bachelor’s Degree in Social Work, Psychology or related field
- Experience working in the mental and behavioral health field
- Professional and courteous communication (verbal and in writing) and knowledge of and sensitivity to culturally diverse and special needs populations.
- Valid GA driver’s license with clean driving record
- Satisfactory results of Criminal background check
