Job Summary

A company is looking for a Health Coach (1099).

Key Responsibilities

• Provide motivational support and educational communications to members managing chronic conditions via Zoom
• Utilize motivational interviewing and coaching techniques to promote lasting behavior changes in nutrition, sleep, movement, and stress management
• Document member visits and interactions accurately in the electronic medical record (EMR)

Required Qualifications

• Active certification in health coaching or related fields, with NBC-HWC preferred
• Expertise in chronic conditions such as diabetes, hypertension, high cholesterol, and obesity
• 5+ years of direct client care experience, ideally in clinical or telehealth settings
• Comfort with digital tools, including EMRs and coaching platforms
• Experience in virtual care, including chat, phone, and video
