Job Summary

A company is looking for an Analyst, Case Management Specialist (Remote).

Key Responsibilities

• Engage with members telephonically to coordinate care, dedicating 50-75% of the day to this task
• Develop individualized care plans in collaboration with members to address their needs
• Document care management activities and collaborate with the Interdisciplinary Care Team to overcome barriers to care

Required Qualifications

• 2+ years of experience in a health-related field
• 2+ years of customer service experience
• Proficient in Microsoft Office Suite
• Access to a private, dedicated workspace for conducting work
• Associate's degree in a health care-related field or relevant experience
