Bilingual Remote Care Coordinator (LPN)

Job Summary

A company is looking for a Bilingual Remote Care Coordinator (LPN/LVN/RN) in the Alaska Time Zone.

Key Responsibilities

• Conduct monthly phone calls with patients and related parties to ensure care needs are met
• Create and revise personalized care plans in coordination with healthcare providers
• Identify social determinants of health and collaborate with community resources to address patient needs

Required Qualifications

• At least 1 year of experience as an LPN with an active professional license
• Exceptional verbal and writing skills in English and Spanish
• Experience with multiple EMR/EHR systems and proficiency in Microsoft Office and Google Suite
• Highly self-motivated and organized, capable of working independently
• Experience in case management or care coordination is a plus
