Responsibilities

• Provide personal care services to clients including assistance with bathing, grooming, and dressing.
• Aid clients with mobility, including walking and transfers.
• Assist with meal preparation and feeding as needed, ensuring nutritional needs are met.
• Administer medication reminders and document accordingly based on treatment plans.
• Perform light housekeeping duties to maintain a clean and safe living environment.
• Monitor and report changes in the client's condition to the supervising nurse or case manager.
• Engage clients in social activities and provide companionship to promote mental well-being.

• Must possess a valid Certified Home Health Aide (CHHA) certification.
• Previous experience as a Home Health Aide or in a similar role is preferred.
• Strong communication skills, both verbal and written.
• Ability to follow care plans and instructions accurately.
• Compassionate and patient demeanor when working with clients.
• Reliable transportation to travel to client locations as needed.
• Availability to work flexible hours, including evenings and weekends.
