Key Responsibilities

• Provides telephonic and virtual outreach and engagement services to eligible members
• Collects and documents member information, including consent and emergency contacts
• Initiates the intake process and communicates urgent needs to care teams

Required Qualifications

• 1 year of experience in a similar role
• Bilingual preferred
• Familiarity with data management and tracking systems
• Ability to work collaboratively with care teams
• Experience in providing customer support or outreach services
