Job Summary

A company is looking for a Community Resource Navigator - Community Health Worker.

Key Responsibilities

• Collaborates with patients, healthcare professionals, and community organizations to address social care needs
• Conducts screenings and assessments to identify resources for patients with unmet needs
• Provides education about available community resources and maintains relationships with patients and organizations

Required Qualifications

• Bachelor's degree in public/community health or social work, or an Associate degree with relevant experience
• Completion of SCCHW Certification within one year of hire or other Community Health Worker training
• Valid driver's license and acceptable motor vehicle record
• Ability to work independently and manage time effectively
• Available to work occasional evenings and weekends
