Job Overview:

Consulting Connection Services is seeking a qualified and motivated Benefits Planner and Housing Coordinator to support individuals with disabilities in navigating complex benefit systems and securing appropriate housing supports. This dual-role position is instrumental in promoting financial independence, community integration, and housing stability for the individuals we serve.

The ideal candidate will possess certification in benefits planning from VCU or Cornell and demonstrate a strong commitment to advocacy, client education, and cross-agency collaboration.

Key Responsibilities:

A. Benefits Planning (CWIC / WIPA Duties)

  • Provide comprehensive benefits counseling to individuals receiving Social Security Disability benefits (SSI/SSDI), focusing on the impact of employment on public benefits.
  • Educate individuals, families, and support teams about work incentives, such as PASS Plans, IRWEs, and subsidies.
  • Develop individualized work incentive plans and benefits summaries based on SSA regulations.
  • Assist clients in understanding eligibility, timelines, and procedures for various federal, state, and local programs (e.g., Medicaid, Medicare, SNAP, HUD).
  • Maintain strict confidentiality of client data and comply with SSA and grant documentation requirements.
  • Collaborate with Support Coordinators, employment specialists, and external agencies to align benefits planning with vocational goals.
  • Utilize case management systems and reporting tools required by VCU, Cornell, or SSA-affiliated grants.

B. Housing Coordination (DBHDS-Focused)

  • Provide clear overviews of DBHDS housing resources to individuals and their families.
  • Collaborate with Support Coordinators and planning teams to gather necessary information and documentation to complete DBHDS Housing Resource referrals.
  • Obtain appropriate consents for information sharing with DBHDS, Support Coordinators, and related parties.
  • Submit and manage housing referrals in compliance with DBHDS processes, including timely updates and corrections.
  • Communicate outcomes of housing applications to individuals, their teams, and families, and guide them through next steps in securing or maintaining housing.
  • Monitor and support individuals' progress with housing placement and coordinate resources to promote long-term housing stability.
    • Required Credential: CWIC Certification through VCU or Cornell University.
    • Bachelor’s degree in Social Work, Human Services, Rehabilitation, Public Policy, or related field (Master’s preferred).
    • At least 2 years of experience working with individuals with disabilities, particularly in benefits counseling or housing support roles.
    • Deep understanding of Social Security Administration programs, work incentives, and Medicaid/Medicare interactions.
    • Familiarity with DBHDS housing initiatives and resources.
    • Excellent interpersonal, organizational, and communication skills.
    • Ability to manage confidential information and work independently while coordinating with multiple stakeholders.

Compensation Rate: $22-$25 per hour**
