Washington Licensed Care Manager

Job Summary

A company is looking for a Care Manager for the Suicide Prevention Program.

Key Responsibilities

• Assist with outreach, engagement, and continuous support for members, providing regular check-ins and personalized care
• Develop and coordinate community resources to address medical, behavioral, and social service needs
• Assess risk, perform crisis interventions, and ensure safety through tailored safety plans

Required Qualifications

• Bachelor's degree in psychology, social work, or a related behavioral health discipline from an accredited institution
• Master's degree in a related field is preferred
• At least 2 years of experience in a related role, preferably with at-risk adolescents and young adults
• Crisis intervention training or similar certification is required
• Knowledge of local resources, mental health, and substance abuse disorders
