Job Summary

A company is looking for a Clinical Case Manager - Behavioral Health.

Key Responsibilities

• Coordinates behavioral health assessments and determines appropriate social support systems for patients
• Develops personalized treatment plans and monitors patient progress towards goals
• Implements resource coordination systems for outpatient counseling, support groups, and other services

Required Qualifications

• 3+ years of clinical behavioral health experience
• Active License Clinical Social Work (LCSW) or License Professional Counselor (LPC) in Virginia
• Master's degree required
• Must be available to work Monday-Friday, 8:00am-5:00pm EST
