Job Summary

A company is looking for an Online Referral Specialist in Home Health.

Key Responsibilities

• Monitor, review, and respond to inquiries on the company website
• Evaluate incoming requests to determine eligibility for home health/hospice services and coordinate referrals
• Communicate with home health Intake Coordinators to ensure complete referral information and assist in scheduling evaluations

Required Qualifications

• Associate's degree or equivalent
• Two (2+) years' administrative experience in a healthcare environment
• Current, unencumbered license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse
• Reliable internet service and ability to work from home with minimal disruption
