Job Summary

A company is looking for a Telehealth Physician to support the Social Determinants of Health program.

Key Responsibilities

• Conduct comprehensive evaluations and develop personalized care plans for patients in the Social Determinants of Health program
• Collaborate with multidisciplinary teams and provide clinical direction to ensure comprehensive patient care
• Educate patients on the use of remote monitoring devices and analyze health data to improve patient outcomes

Required Qualifications

• Medical Degree (MD or DO) from an accredited medical school
• Certification in Family Medicine, Internal Medicine, or a closely related field
• 3-5 years of clinical experience, preferably in Family Medicine or Primary Care
• Valid, unrestricted medical license in one or more specified states
• NPI Number and ability to secure malpractice insurance
