Job Summary

A company is looking for a System Case Management and Care Transitions Educator.

Key Responsibilities

• Develop, implement, and evaluate evidence-based educational programs for Nursing and Social Workers
• Provide enterprise-wide inpatient Case Management education and assist in implementing standardized education components
• Facilitate ongoing staff development, annual competence assessments, and performance improvement initiatives

Required Qualifications

• Bachelor's degree in nursing or a Master's degree in Social Work required
• Minimum of five years of applied Case Management experience as an RN/Case Manager
• Preferred Master's degree for nursing candidates and experience in an education role
• Current South Carolina licensure as a social worker or registered nurse required
• Certifications in Case Management (ACM, CCM) required within 6 months of hire
