Job Summary

A company is looking for a Clinical Care Specialist.

Key Responsibilities

• Manage and coordinate Chronic Care Management services for patients in long-term care facilities
• Create and update individualized Care Plans and monitor patient health metrics
• Conduct care coordination and provide education to patients and their families

Required Qualifications

• Registered Nurse (RN) with an active compact license in the applicable state
• Five years of nursing experience required
• Experience with chronically ill patients preferred
• Telephonic case management or care coordination experience preferred
• Strong computer skills and reliable internet connection required
