Remote LPN Case Manager

Job Summary

A company is looking for a Case Manager LPN (Licensed Practical Nurse).

Key Responsibilities:

• Maintain remote access to required EHR systems
• Conduct data mining to close quality gaps within EHR systems
• Contact provider offices to request medical records

Minimum Qualifications:

• Active Licensed Practical/Vocational Nurse (LPN/LVN) license
• At least 1 year of experience in provider outreach and/or EHR system navigation
• At least 1 year of experience with HEDIS /STARs data abstraction
• At least 2 years of experience in data analysis and quality chart reviews
• Proficiency in Microsoft Office applications, including Word and Excel
