Registered Nurse Care Manager

Job Summary

A company is looking for a Care Manager (RN).

Key Responsibilities

• Evaluates member needs and barriers to care, recommending plans for optimal outcomes
• Develops and collaborates on ongoing care plans, addressing unmet needs with providers and resources
• Monitors member status and progress, updating care plans as necessary to meet goals

Required Qualifications, Training, and Education

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• Registered Nurse (RN) State Licensure and/or Compact State Licensure required
• Strong background in Case Management and Behavioral Health Management
