Job Summary

A company is looking for a Program Support Specialist.

Key Responsibilities

• Recover no contacts and disenrollment, complete Patient-Reported Outcome Measures (PROMS), and schedule coaching sessions
• Conduct outreach to address HEDIS-related gaps and collaborate with coaches and quality specialists
• Analyze program performance data and contribute to quality improvement initiatives

Required Qualifications

• Bachelor's degree required; RN, LPN, or Nationally Certified Coach preferred
• 3+ years of experience in health, wellness, or community health settings required
• Equivalent education and/or experience may be considered in lieu of a degree
