Job Summary

A company is looking for a Case Manager RN (Remote).

Key Responsibilities

• Develop proactive care plans to enhance member wellness and outcomes
• Conduct evaluations of member needs using clinical tools and data
• Collaborate with interdisciplinary teams to address health and social barriers

Required Qualifications

• Active and unrestricted RN licensure in Pennsylvania or compact licensure
• Minimum of 3 years of clinical practical experience
• Minimum of 2 years of case management or home health care coordination experience
• Associates Degree or Nursing Diploma required; Bachelor's Degree preferred
• Certified Case Manager is a plus
