Job Summary

A company is looking for a Patient Navigator to facilitate onboarding and engagement for Managed Medicare Risk Members.

Key Responsibilities

• Initiate contact with new members and provide an overview of available services and benefits
• Document interactions and outcomes in Epic, ensuring accurate member data and attribution
• Collaborate with care coordinators to ensure timely member outreach and follow-up

Required Qualifications

• High School Diploma/GED or higher
• 2+ years of experience in analyzing and solving customer problems
• 2+ years of experience in the healthcare industry
• Intermediate computer proficiency, including Microsoft Outlook, Excel, and Word
• Access to a designated quiet workspace and high-speed internet connection
