Job Summary

A company is looking for a Patient Care Coordinator who manages in-home and virtual appointments for providers and clinical staff.

Key Responsibilities:

• Coordinate and schedule appointments for providers and clinical staff, both in-person and virtual visits
• Utilize scheduling software to efficiently manage provider and clinical staff schedules
• Collaborate with operational and clinical teams to identify and address scheduling gaps and urgent needs

Required Qualifications:

• High school diploma or GED
• 2+ years work experience in a healthcare setting
• Preferred: Call center or scheduling experience
• Preferred: Experience with patient communication protocols
• Preferred: Experience in a value-based healthcare setting
