Job Summary

A company is looking for a Care Navigator- LPN/LVN to provide remote patient care and support.

Key Responsibilities

• Manage physiological markers such as blood pressure, weight, and blood glucose
• Conduct monthly wellness calls and lead collaborative discussions to define health goals
• Provide preventive health education and manage patient messaging and alerts

Required Qualifications

• Unrestricted LPN/LVN license in a compact state and Oregon
• 1-3 years of clinical experience in patient or disease management
• Experience with telephonic care management and virtual patient visits
• Ability to work independently and adapt to different provider practices
• Spanish speaking is a plus
