Job Summary

A company is looking for a Nurse Case Manager to support patients in various healthcare settings.

Key Responsibilities

• Independently manages complex healthcare cases, assessing patient needs and educating members on treatment options
• Facilitates interventions based on care treatment plans and coordinates services with community resources
• Maintains documentation in compliance with company and regulatory guidelines

Required Qualifications

• Graduate of a registered nursing program approved by the Ohio State Nursing Board; Bachelor's Degree preferred
• 3 years of experience as a Registered Nurse with clinical and case management experience, preferably in health insurance
• Current State of Ohio unrestricted Registered Nurse license; multiple state licensure preferred
• Certified Case Manager (CCM) or other industry-recognized certification preferred (required within 3 years of hire)
• In-depth knowledge of health insurance benefits and health coaching techniques
