Key Responsibilities

• Develops and assesses ongoing care plans while collaborating with providers to address member needs
• Coordinates care between members, families, and care providers to ensure person-centered services
• Monitors and updates care plans based on member status and collaborates with stakeholders to meet goals

Required Qualifications

• Active New York State RN License
• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 4 - 6 years of related experience
• Experience with care management and member services
• Knowledge of state and federal regulations related to healthcare
