Job Summary

A company is looking for a Senior Care Manager (RN).

Key Responsibilities

• Develops and assesses ongoing care plans and collaborates with providers to identify necessary resources for members
• Coordinates care between members, families, and care providers to ensure person-centered services
• Monitors care plans and member progress, revising plans as necessary to meet member goals

Required Qualifications

• Active New York State RN License
• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 4 - 6 years of related experience
• Knowledge of state and federal regulations regarding healthcare
• Experience in care management and collaborating with healthcare providers
