Job Summary

A company is looking for an RN Specialist in Complex Case Management.

Key Responsibilities

• Engages members with high-cost, complex medical conditions through telephonic outreach and clinical assessments
• Collaborates with the interdisciplinary care team to develop and monitor comprehensive plans of care
• Identifies cost-saving opportunities and facilitates referrals to relevant resources

Required Qualifications, Training, and Education

• Current MN RN license with no restrictions
• 5 years of relevant clinical care experience
• CCM Certificate or ability to obtain within 3 years of starting
• Experience in managed care preferred (2+ years)
• Proficiency in computer applications
