Key Responsibilities

• Receives referrals and conducts comprehensive clinical assessments to identify member needs
• Collaborates with members and families to develop and monitor care plans that promote positive clinical outcomes
• Facilitates referrals to community resources and ensures accurate documentation of member data

Required Qualifications

• Registered Nurse with psychiatric experience or a licensed behavioral health professional
• 3 years of relevant direct clinical care experience
• CCM Certification or ability to obtain within 3 years
• Flexibility to work varied hours
• Computer application proficiency
