A company is looking for a Michigan Registered Nurse Care Coach.

Key Responsibilities

• Utilize care management software to call Medicare patients with chronic conditions monthly
• Build rapport and coach patients on self-management strategies to improve health
• Implement and update the Plan of Care, connecting patients with necessary community resources

Required Qualifications, Training, and Education

• Current, unrestricted Michigan License
• Proficiency with electronic health records and web-based applications
• 3+ years' experience as a Registered Nurse
• Self-directed and able to work independently with minimal supervision
• Good with technology and eager to learn new software
