Michigan Licensed Ambulatory Case Manager

Job Summary

A company is looking for an Ambulatory Case Manager (RN or LMSW) for a remote role.

Key Responsibilities

• Provide telephonic case management to primary care patients
• Assess patient needs, set goals, and build tailored care plans
• Identify and connect patients to community and health resources

Required Qualifications

• Active Michigan RN license or LMSW license in good standing
• Bachelor's in Nursing or Master's in Social Work required
• Minimum 3 years of clinical experience and 3-5 years in telephonic case management
• Familiarity with motivational interviewing, goal setting, and care coordination
• Comfortable working independently in a fast-paced virtual environment
