Georgia Licensed Nurse Care Manager

Job Summary

A company is looking for a Registered Nurse Care Manager II to provide virtual oversight of patients in home-based Remote Patient Monitoring programs.

Key Responsibilities

• Establishes vital sign parameters and ensures documentation supports quality management and billing
• Collaborates with care team members and follows escalation protocols for changes in patient condition
• Assesses patient needs, provides education, and recommends appropriate programs or community resources

Required Qualifications

• Associates Degree in Nursing Science required; Bachelor's Degree preferred
• Compact State Licensure required (RN Compact) and current RN Licensure in Georgia
• Three (3) plus years of clinical experience, preferably in acute or ambulatory care
• Experience working in a fully virtual environment and with senior populations in health service settings is a plus
• Proficient in nursing processes and EHR/CMS systems
