Job Summary

A company is looking for a Care Manager (RN).

Key Responsibilities

• Develops and facilitates complex care management activities and personalized care plans for members
• Coordinates care between members, families, and care providers to ensure appropriate services
• Monitors member progress and revises care plans as necessary to meet their goals

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
